  • That is difficult to answer, due to the inherent (and possibly purposeful) complications of the insurance industry. You’ll need to know if you’ve met your deductible and if you have a copay. If you have not met your deductible, the cost varies from $125-$300/ visit through insurance.

    4. Calculate Copayments and Coinsurance: Identify if your plan requires a copayment per visit (a fixed fee) or coinsurance (a percentage of the cost). Multiply the expected number of sessions by these amounts to estimate your out-of-pocket expenses.

    5. Contact Your Insurance Provider: Call your insurance company directly to verify your benefits for physical therapy. Ask about in-network providers, coverage limits, costs per session, and any pre-authorization requirements.

    6. Factor in Additional Costs: Consider potential costs like evaluations, specialized treatments, or equipment that might not be fully covered. Most insurance providers do not cover NEUBIE Therapy or Dry Needling Therapy.

  • Plans that do need Prior Authorization:

    Workers Compensation Claims require Prior Auth. We will need the following:

    1. Your claim number

    2. Your employer

    3. Your Worker’s Comp Insurance Provider

    4. Date of Injury

    5. Case Manager/ Adjustor Name, Email and Phone number

    6. Authorization Number if you have it.

    Alignment: Requires referral from your MD

    All Well: No referral required, we can obtain authorization

    Banner UFC: No referral required, we can obtain authorization

    Humana: No referral required, we can obtain authorization

    Tricare: Requires a referral from your MD

    Veteran’s Affairs: Requires initial auth from the VA

    4. Understand Consequences: Without prior authorization when required, your insurance may deny coverage, leaving you responsible for the full cost of treatment.

    Diagnostic imaging is not required before your initial physical therapy appointment. Physical therapists are highly trained to evaluate your condition through a thorough clinical examination and patient history. Starting therapy early can often speed recovery, reduce pain, and improve function without waiting for imaging results. If imaging is necessary, your therapist will refer you for appropriate tests based on specific clinical findings. This approach minimizes delays and allows you to begin personalized treatment promptly.
